Please submit your request via email to support@permissions.zendesk.com with the following information:

• Include "Hal Leonard Artwork" in the subject line
• Your full name, school or organization name, street mailing address, telephone number, and e-mail address
• Title of product that includes the artwork you wish to use
• Details of how the artwork will be used, including number of copies/units produced, if they will be sold, etc.
• A scan of the artwork you wish to reproduce, attached as a PDF or image file
